Care Transformation Organization (CTO)

BNWS has been selected to become a Care Transformation Organization (CTO) to assist practices transitioning to the new model of care in Maryland called the Maryland Primary Care Program (MDPCP).

The Maryland Department of Health states that a CTO is defined as:

“…an entity that hires and manages an interdisciplinary care management team capable of furnishing an array of care coordination services to Maryland Medicare beneficiaries attributed to Participant Practices.”

About Our Company

BNWS is a small healthcare, wellness and care coordination organization serving a growing number of patients. Our team of healthcare providers has over 100 cumulative years of experience working in outpatient, small practice settings. As such, we are intimately familiar with the day-to-day challenges that small practices face.

We distinguish ourselves from larger hospital conglomerations in Maryland in that we focus on partnering with patients as well as creating connections and finding resources in the communities that we serve to provide the most well-rounded, patient-centered care for our patients.

    BNWS will focus on assisting small practices with 1-4 providers throughout Maryland who would like to become a fully-fledged MDPCP.

    Smaller medical practices can get overwhelmed by competition with the larger hospital systems in Maryland. As a CTO, BNWS will be able to provide those resources that small practices need to keep up with the ever-changing healthcare system and ultimately ensure practices like these continue to thrive in the face of an increasingly consolidated health care market.

  • What we will do for you

    Our group has been growing and refining our care coordination services within our own clinical practice and we would like to bring our knowledge and experience to your practice. We provide care coordination, staff training, and can help you set up a workflow structure to support the transition to the new Maryland model of healthcare delivery .

    BNWS CTO will allow for small practices to transform the care for their patients while also maintaining financial independence and sustainability. BNWS will also be able to provide training for care coordinators and chronic condition case managers on techniques such as motivational interviewing, chronic disease self-management, and panel management. CTO will work with partner practices to transform to a patient-centered, team-oriented approach to healthcare.
